The Black Galaxies won 2-0 in the first leg, but Zulkifilu Muhammed and Chijioke Akuneto scored in the second half of the return leg to send the game to penalties.
After surrendering their first leg lead, the Ghanaian team did not dwell on the disappointment but converted all their spot kick to win 5-4 on penalties.
This is the first time the Black Galaxies have qualified for CHAN since the 2014 edition in South Africa.
As for the home-based Super Eagles, they have now missed back-to-back edition of the Africa Nations Championship.
After a goalless first half, the home-based Super Eagles pulled a goal back in the 76th minute through Muhammad.
